Welcome to Aksyr Medical Practice
At Aksyr Medical Practice the doctors and nurses take pride in offering the highest standard of patient-centred healthcare. We run many clinics for chronic disease care and offer a wide variety of other medical services such as antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.
In addition to everything you need to know about the practice you will also find a wealth of health-related information in the menu on the right hand side. Please have a look around and do send us some feedback if you like.

- You can join our Patient Group by sending your email address to email@example.com. and we will send you the survey to complete

Patients aged 16 and above are now able to make or cancel their appointments on line, to order their repeat medication and to view a summary of their medical record. To find out more, please read the Patient Online Leaflet.pdf and Patient Online Frequently asked questions.pdf
If you wish to use these services please complete the Patient Online RegistrationForm.docx and bring it to the Practice. You will need to provide ID to confirm you are the patient in question and we will issue you with an unique ID number and the details of how you can register for this service. Alternatively, you can speak to a receptionist, or if you have any further questions email the Practice at firstname.lastname@example.org or write to the Practice.
We hope that this helps patients to access our services and manage their care more effectively at a time that is convenient to them. It should also reduce the number of calls to the Practice and hopefully reduce delays on the telephones.
7 Day Access To GPs
The practice are working with the other local practices in Harness to provide additional GP appointments between 6-9pm Monday to Friday and 9am to 3pm Saturday and Sundays. If you would like an appointment with the hub please speak to a receptionist who will be happy to help you.

Opt out form
The practice are participating in the North West London whole system integration programme. This includes modelling the new model of care using anonymised data extracted from the practice clinical data base. The same process is happening with hospital, community and social care information to help provide more joined up care to patients. The leaflets above provide information on the data extraction process and an opt out form for patients who do not wish their medical record to be included in the process. The practice take their responsibility for the protection of patient information very seriously and ensure we have robust systems to maintain patient confidentiality at all times. NAMED ACCOUNTABLE GP
Over the next few weeks the surgery will be informing patients of who their "named accountable GP" is, as required by our GMS 2015-16 contract( see page 9 of 101). THIS WILL IN NO WAY AFFECT THE GP YOU WISH TO CONSULT WITH. We have to do this like every other surgery at the first appropriate opportunity.
EMAIL & SMS - to try to reduce the adminstrative burden on our GPs and staff we will try to inform our patients via text and email where appropriate. We have some reservations that this might cause some confusion to our patients and detract from actual patient care, but we have a contractual obligation to undertake this.
We operate a pooled list with our GPs working together, but if we need to, the named accountable GP will take lead responsibility for the coordination of all services required under the contract and ensure they are delivered to each of their patients where required. Again this will not change the GP you wish to consult with.
If you want to know before this, who your "named accountable GP" is, please ask any practice member or clinician and they will be able to tell you from the clinical records system.
Brent Health APP
The app's three main purposes are to communicate health news, help residents find local services, provide healthcare information.
Users can also book GP appointments, order repeat prescriptions, access summary medical records and use the app as a symptom checker.
