AKSYR Medical Practice would like to take this opportunity to invited all the patient for our Patient Participation Group (PPG) Meeting on WEDNESDAY 5th JULY 2017 at 6:30PM in our practice.

 The aim and importance of having such a group will be to bridge communication between patients and the practice and therefore look forward to seeing more patients joining.

This is a great opportunity for patients to share ideas, your experiences and make suggestions on how the surgery can provide an even better service to all our lovely patients going on.

Welcome to Aksyr Medical Practice

At Aksyr Medical Practice the doctors and nurses take pride in offering the highest standard of patient-centred healthcare.  We run many clinics for chronic disease care and offer a wide variety of other medical services such as antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

In addition to everything you need to know about the practice you will also find a wealth of health-related information in the menu on the right hand side. Please have a look around and do send us some feedback if you like.

We are always interested in how we are doing.  Here's how you can let us know:

  • Some of you may receive a National GP Patient Survey which you should complete and return as soon as possible so we can get your views
  • Go through to the NHS Choices Website
  • We have a survey available at Reception you can complete
  • You can join our Patient Group by sending your email address to aksyrpractice@nhs.net. and we will send you the survey to complete

Our own survey is based on the outcome of our decisions from our Patient Group meeting.

  • Keep your appointment, or let us know as soon as possible if you can't attend
  • Only use the out-of-hours services for urgent conditions which can't wait until the next day or over the weekend.
  • Be nice to our staff! They do their best for you and respond very well to "please" and "thank you"
  • Let us know if you have any suggestions or cause for complaint as soon as possible
  • Let us know when we have done well

 GP & Staff Training

To maintain a high standard of care, all members of the Practice Teams receive regular training and updating

 

The quality of the service is achieved through supervision of staff and support by regular clinical multidiscipline team meetings, staff training and development and reporting and review of all incidents, and by review of complaints

 

 


 

Register for Patient Access

Patients aged 16 and above are now able to make or cancel their appointments on line, to order their repeat medication and to view a summary of their medical record.   To find out more, please read the Patient Online Leaflet.pdf and Patient Online Frequently asked questions.pdf    

If you wish to use these services please complete the Patient Online RegistrationForm.docx and bring it to the Practice.  You will need to provide ID to confirm you are the patient in question and we will issue you with an unique ID number and the details of how you can register for this service.  Alternatively, you can speak to a receptionist, or if you have any further questions email the Practice at aksyrpractice@nhs.net or write to the Practice. 

We hope that this helps patients to access our services and manage their care more effectively at a time that is convenient to them.  It should also reduce the number of calls to the Practice and hopefully reduce delays on the telephones. 

7 Day Access To GPs

The practice are working with the other local practices in Harness to provide additional GP appointments between 6-9pm Monday to Friday and 9am to 3pm Saturday and Sundays.  If you would like an appointment with the hub please speak to a receptionist who will be happy to help you.

Opt out form

The practice are participating in the North West London whole system integration programme.    This includes modelling the new model of care using anonymised data extracted from the practice clinical data base.  The same process is happening with hospital, community and social care information to help provide more joined up care to patients. The leaflets above provide information on the data extraction process and an opt out form for patients who do not wish their medical record to be included in the process.    The practice take their responsibility for the protection of patient information very seriously and ensure we have robust systems to maintain patient confidentiality at all times.   If you would like to discuss this further please email the practice on BRECCG.AksyrGeneric@nhs.net.

NHS Health Check

Patients aged between 40 year old and 74 years old and who do not have any chronic disease are eligible for a free health check. For further information please speck to reception.

 

MMR Booster vaccination available for 6 - 36 years old

If you or your child are unsure of whether you have had the MMR booster please contact reception.

 

Chlamydia Screening

 

We offer free chlamydia screening (testing) to all men and women aged between 16 - 24.

Most people don't know they have chlamydia and that it can cause pain and infertility if left untreated. 1 in 10 people have chlamydia - but not all have symptoms. Chlamydia testing does not require booked appointments, examinations or blood tests- just a simple urine test.  Please enquire at reception

Carers

DO YOU LOOK AFTER SOMEONE WHO IS ILL, FRAIL, DISABLED OR MENTALLY ILL?

Do you look after someone who is ill, frail, disabled or mentally ill? If so, you are a carer.  We are interested in identifying carers, especially those people who may be caring without help or support.  We know that carers are often “hidden” looking after a family member or helping a friend or neighbour with day to day tasks and may not see themselves as a carer.

We feel that caring for someone is an important and valuable role in the community, which is often a 24-hour job that can be very demanding and isolating for the carer.  We further believe carers should receive appropriate support by way of access to accurate information on a range of topics such as entitlement to benefits and respite care and not least, a listening ear when things get too much.

 NAMED ACCOUNTABLE GP

Over the next few weeks the surgery will be informing patients of who their "named accountable GP" is, as required by our GMS 2015-16 contract( see page 9 of 101).   THIS WILL IN NO WAY AFFECT THE GP YOU WISH TO CONSULT WITH.   We have to do this like every other surgery at the first appropriate opportunity.   

EMAIL & SMS - to try to reduce the adminstrative burden on our GPs and staff we will try to inform our patients  via text and email where appropriate.    We have some reservations that this might cause some confusion to our patients and detract from actual patient care, but we have a contractual obligation to undertake this.

We operate a pooled list with our GPs working together, but if we need to, the named accountable GP will take lead responsibility for the coordination of all services required under the contract and ensure they are delivered to each of their patients where required.   Again this will not change the GP you wish to consult with.

If you want to know before this, who your "named accountable GP" is, please ask any practice member or clinician and they will be able to tell you from the clinical records system.

The app's three main purposes are to communicate health news, help residents find local services, provide healthcare information.
